Please read this Service Agreement carefully before booking any consultation package with Carmen Lei Dietitian. By proceeding with a booking, you agree to the terms outlined below.

1. Consultation Scope

Our dietitian services provide personalised nutrition advice, dietary counselling, and lifestyle guidance. Consultations do not replace medical care and do not include medical diagnosis, medical treatment, or medication management. Clients should consult their doctor or relevant healthcare provider for medical concerns, diagnosis, prescriptions, or changes to medication.

2. Telehealth Limitations

Telehealth consultations are conducted via secure video or phone platforms. While we take all reasonable measures to ensure quality of care, telehealth services may have limitations compared to in-person consultations. Clients must ensure they are in a private, suitable environment during sessions.

3. Payment Terms

Full payment is required at the time of booking unless otherwise agreed in writing. All fees are listed in Hong Kong Dollars (HKD) and are inclusive of applicable taxes. Payments are non-transferable.

4. Cancellation & Rescheduling Policy

Cancellations made more than 72 hours before the scheduled appointment may be eligible for a credit or rescheduling. Cancellations within 72 hours of the appointment or no-shows will forfeit the session fee. Rescheduling requests must be made at least 72 hours in advance.

5.  Consistent Review Time Slot

To support consistency and continuity of care, the time slot selected for your initial consultation will also be reserved as your regular review consultation time slot for the duration of your package. Maintaining a consistent review schedule helps support treatment progress, accountability, and better long-term outcomes. Also because slots can fill up quickly within the week, so any changes to your regular review time must be requested at least 72 hours in advance. Rescheduling is subject to availability. If no suitable alternative time is available within the same review week, the session will be considered missed and will not be refunded or carried forward.

6. Privacy Terms

All personal and health information collected during consultations is handled in strict confidence in accordance with applicable privacy laws. Your data will not be shared with third parties without your explicit consent, except where required by law.

7. Supplement Access Conditions

Access to clinical grade supplements recommended by our dietitian is subject to a valid consultation and professional assessment. Supplements are not available for purchase without a prior or concurrent consultation. Recommendations are made solely based on individual clinical need.

8. Agreement
